Qualifying condition
i) Where a member ceases employment on medical grounds based on Medical Board’s recommendations and employer’s satisfaction.
Mode of granting disability benefit
ii) If the member has at least 120 months contributing period is granted;
• “Commuted Pension Gratuity” (CPG) (lump sum) immediately on ceasing employment; and
• Monthly pension regardless of his or her age for the rest of his or her life;
iii) If such member has less than 120 months contributing period
• Is granted gratuity immediately on ceasing employment.

Documents required
i) Benefit Claim form duly filled by the employer; (See Page 25)
ii) Certified proof (e.g. Board/Management Resolutions) from the employer stating the decision to retrench, retire, or remove the member from employment;
iii) Two passport-size photographs of the member;
iv) Bank details – Name and Account, for remitting monthly old-age- pension
